• In-Home Services Coordinator

    Enjoy helping others? Want to make an impact on the Senior Citizens in our community? Meals on Wheels- OAAFC is seeking a part-time In-Home Service Coordinator to partner with outside providers to fill our client’s assistance needs.

    ESSENTIAL JOB FUNCTIONS
    Responsible for the Medical Guardian Personal Emergency Response System (PERS) Program, In-Home Repair Program, and Adaptive Equipment Program, and all job duties related to them, to include, but not limited to:

    • Enter and maintain all documentation for each program into program applications
    • Complete all paperwork assigned for each program
    • Report service data for each program
    • Schedule and perform installation and removal of PERS
    • Ensure monthly testing for the PERS units is performed and coordinate any problem solving related to the PERS service
    • Coordinate and schedule repairs with internal maintenance or external providers for In-Home Repair Program, as well as follow-up with client to ensure satisfaction of repair
    • Keep Adaptive Equipment room organized, labeled, and accessible
    • Clean and sanitize returned equipment to ensure it is in working, safe status
    • Assess client request and ensure availability of equipment
    • Schedule and perform delivery and removal of equipment
    • Other duties as assigned to ensure the Agency Mission is fulfilled
     
     
    REQUIREMENTS
    Minimum of 2 years in human services, geriatrics, or health care setting in a similar role
    Associate’s degree in Social Services or related program, preferred
    Organized, attention to detail, and experience with computers
    Ability to lift and carry up to 30 pounds
    Possess a valid Ohio driver’s license, vehicle insurance, and safe driving record
