• Service Navigation Manager

    Are you passionate about helping older adults? Do you want to make a positive impact on the lives of older adults in our community? The Senior Hub-Meals on Wheels of Fairfield County is seeking a Service Navigation Manager who possesses strong interpersonal communication and problem-solving skills. In this role, you will assist older adults by creating care plans and navigating services available within the community to bring about positive outcomes.

    The Senior Hub-Meals on Wheels of Fairfield County is a large, 501c3 nonprofit organization serving Fairfield County.  The agency serves as the county’s office on aging providing programs and services to improve health, connection, and independent living for those 60 and older living in Fairfield County.
     
    ESSENTIAL JOB FUNCTIONS

    • Support older adults presenting complex needs through case management
    • Develop, implement, and monitor care plans working with the individuals, families, social supports, and health care professionals
    • Provide linkage, referral, and advocacy services to various resources
    • Administrative duties, including paperwork, documentation, and reporting
    • Oversee and maintain the agency’s Caregiver Support Program
    • Implement and maintain compliance needs
     
    REQUIREMENTS
    Bachelor’s degree in social work, psychology, human services, or related field
    A minimum of 2 years’ experience in human services, geriatrics, or health care setting or similar role
    Hold a valid driver’s license and good driving record
    Active vehicle insurance
